In general, most companies have adopted formal procedures and systems to deal with incidents that occur within their facilities or within their jurisdictions. These might include a notification procedure, an incident investigation system, and an action items implementation and follow-up process. What most companies do not have is a formal system of learning from other people’s incidents; incidents that are reported in the press and investigated by independent third parties (sometimes government or quasi-government institutions).
When an incident is reported in the media everyone is naturally curious as to what happened. But if we want to learn from the incident we must go beyond simple curiosity.
